Workshop Registration
Two easy ways to register:
Register and pay online:
Instructions: On the form below, 1) Enter
workshop number (1, 2, or 3) and then the young man's name and birth
date. 2) Click on the PayPal icon where you will be prompted to
enter your credit card or echeck payment information. (It's easy and
secure.)
Or register via email (and mail in your payment)
Email all of the following information to register@surepassage.org
:
- Workshop number (1, 2, or 3)
- Name of young man
- Birth date of young man
- Name of father or male mentor
- Address
- Telephone (include home, work, and cell as appropriate)
And send a check (made payable to "Sure Passage") to:
